    I believe I need physical therapy, what should I do?

    You should talk to your physician and together decide if physical therapy would benefit you.  
    Then ask your doctor for a prescription for therapy (they can access one on this site on Medical Providers page.)
    Once you have a prescription call us to schedule.  Please have the following information available when you call:

    1.        Name of referring doctor.
    2.        Insurance carrier
    3.        Member ID number

    How do I know if my insurance will cover physical therapy?

    The reason that we ask you for your insurance information at the time of scheduling your appointment is to verify your eligibility and benefits.  
    When you come in we will inform you of co-pays, deductible, or co- insurance percentage.    Although we do verify benefits, it is always
    advisable that you also check on your insurance benefits. Insurance is billed as a courtesy to our patients; however, it is the patients
    responsiblity to pay all charges regardless of insurance coverage.


    I have made an appointment, what should I bring?

    We will need the following when you come in for your Initial Evaluation:

    1.        If possible go to the Forms & Brochures page on this site and print out and complete Registration Forms.

    2.        You will need to complete the Upper Extremity DASH Forms if we are going to treat your elbow, arm, or hand for ALL insurance carriers,
              including Work Comp.

    3.        Prescription

    4.        Insurance Card(s)

    5.        Picture ID or Drivers License


    How much time should I expect my appointment to last?

    Initial evaluations last 1 hour we ask that you arrive 15 minutes early to complete necessary paperwork.

    I have made an appointment, what should I wear?

    Wear comfortable, loose fitting clothing.


    I am interested in signing up for the Arthritis Aqua Therapy Classes, what should I do?

    Our Aqua Therapy classes are very popular, and usually have waiting lists.  You can call us, or email Maria Hernandez at
